01   Agreement to Terms

Your acceptance of these terms

These Terms of Use govern your access to and use of the Signature Vitality Clinic, LLC website located at signaturevitalityclinic.com and any associated clinical services. Signature Vitality Clinic is operated by Theresa Elabor, MSN, APRN, AGPCNP-C in Houston, Texas.

By visiting our website, booking an appointment, or receiving services from us, you agree to be bound by these terms. If you do not agree, please do not use our site or services. These terms may be updated from time to time; continued use after changes are posted constitutes acceptance.

02   Services

What we offer

Signature Vitality Clinic provides direct-pay medical and wellness services to patients in the greater Houston area and, where permitted, via telehealth throughout Texas. Our services include, but are not limited to:

• Medical weight management and GLP-1 protocol supervision
• IV hydration and vitamin infusion therapy
• Concierge IV services
• Peptide therapy
• Hormonal support and optimization
• Telehealth primary care
• Vitamin B12 and other injection therapies

All services are provided at the clinical discretion of our licensed providers. We reserve the right to decline to provide services when clinically inappropriate or outside our scope of practice.

03   Medical Disclaimer

Not a substitute for emergency care

If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Signature Vitality Clinic does not provide emergency medical services and is not equipped to manage medical emergencies.

Information on our website is provided for general educational purposes only. It does not constitute medical advice and does not create a provider-patient relationship. A provider-patient relationship is established only through a formal clinical intake and mutual agreement to enter care.

Individual outcomes vary. Results described or shown in testimonials reflect specific patients' experiences and are not a guarantee of the results you will achieve. All treatment decisions are made jointly between you and your provider based on your individual health status.

04   Eligibility

Who may use our services

To receive clinical services from Signature Vitality Clinic, you must be at least 18 years of age and physically located in Texas at the time of any telehealth appointment. For in-person services, you must be able to attend our Houston clinic location.

You represent that all information you provide to us is accurate and complete. Providing false or misleading health information may affect the safety and appropriateness of your care and may result in termination of the provider-patient relationship.

05   Telehealth Services

Conditions of virtual care

Telehealth consultations are conducted over secure video and phone platforms. By scheduling a telehealth appointment you consent to the delivery of care through electronic means and acknowledge the following:

• Telehealth has inherent technical limitations; there may be delays or interruptions in transmission
• Your provider may determine that a telehealth visit is not appropriate for your clinical situation and may require an in-person visit instead
• You are responsible for ensuring you are in a private, secure environment during your telehealth session
• You must be located in Texas at the time of a telehealth visit to comply with state licensing requirements

06   Appointments

Scheduling and cancellation policy

Appointments are booked through our online portal. We ask that you provide at least 24 hours notice if you need to cancel or reschedule. Late cancellations and no-shows may be subject to a fee. Repeated no-shows may result in a limitation on future booking privileges.

We make every effort to see patients at their scheduled time. Occasional delays may occur due to clinical necessity. We appreciate your patience and will communicate with you if a delay is expected.

07   Payment

Fees and self-pay terms

Signature Vitality Clinic is a direct-pay practice. We do not bill commercial insurance. Payment is due at the time of service. We accept major credit and debit cards, and many services are eligible for payment with HSA or FSA cards.

08   Intellectual Property

Website content and ownership

All content on the Signature Vitality Clinic website, including text, graphics, logos, photography, and page layouts, is the property of Signature Vitality Clinic, LLC or its content providers and is protected by applicable copyright and trademark law.

You may view and print content from this site for your personal, non-commercial use. You may not reproduce, modify, distribute, or republish any content without our prior written permission.

09   Limitation of Liability

Limits on our responsibility

To the fullest extent permitted by applicable law, Signature Vitality Clinic, its providers, and its staff will not be liable for indirect, incidental, special, or consequential damages arising out of your use of our website or services, including damages resulting from reliance on information provided on this site.

Nothing in these terms limits our liability for acts of professional negligence, which are governed by applicable Texas law and our professional obligations as licensed healthcare providers.

10   Governing Law

Jurisdiction and disputes

These Terms of Use are governed by and construed in accordance with the laws of the State of Texas, without regard to conflict of law principles. Any dispute arising under these terms shall be resolved in the state or federal courts located in Harris County, Texas, and you consent to the exclusive jurisdiction of those courts.
